"The room was damp, and moldy smelling. All the bedding was damp and wet. The pillows were visibly moldy and unclean. It was a struggle to get the staff to provide new and clean and dry bedding. There was no heat source in the room. The hot air blower provided was unusable as had electrical sparks during use.
