We did not and it cost me $214 (aud) for 2 adults and 1 child, we didn't know this ahead of time.
The hot springs / baths are a no cloths zone, kids 6 and older and men and woman are just all in the same water together naked, nothing against there culture it just wasn't for me.
There are claw machines absolutely everywhere in Japan so if your taking kids be sure to always have some coins on you, they range for 100¥ to 500¥
